2017 Annual Conference
REGISTRATION & PAYMENT OPTIONS
Attendance to this meeting is by invitation only. To register online for this event, you need to have a CII web site login and password. (Only personal accounts enable you to register or purchase on the CII site. Please do not use a corporate or generic account.)
If you do not currently have a personal account, please sign up for one and request an invitation to this event by contacting Jenny Bien at jenny.bien@cii.utexas.edu.
Register online if you are using a credit card(s) for payment; go online and order/pay for the events you want to attend, then checkout. If you would like to use a Second Credit Card for optional events: Please complete the first registration round and check out, then re-enter the registration site and check “make changes to your registration” and order/ pay for the other events with second credit card (you will only be billed for the new add-on events). The option to print out a receipt is available after each checkout.

CII Spouses that do not have access to the CII log-in may register for the Spouse program or tours by contacting Debbie Samilpa at 512.232.3017 or email at conference@cii.utexas.edu.
CANCELLATION DATE FOR ALL EVENTS AND ACTIVITIES: April 30, 2017
- FULL REFUND of guest registration fee(s) if cancellation received by April 30.
- NO PARTIAL REFUNDS
- NO REFUND of fee(s) if cancellation received after April 30.
CANCELLATION POLICY
- Notification of cancellation must be received by CII in writing via e-mail, fax, or mail.
- Registrations received without pre-payment by check or credit card and/or “no shows” are also subject to the cancellation policy.
- The completion and return of this registration form acknowledges that you understand and abide by these terms.
- SUBSTITUTIONS ARE ALWAYS ALLOWED.
